Confidentiality

I will keep all information about our sessions (including the fact that you are or have been in therapy with me) strictly confidential, in accordance with the Ethical Principles of the American Psychological Association and HIPAA regulations. These regulations also protect the privacy of all communications between us.

Forms

All potential clients must fill out and submit to my email the following three forms:

• Consent for Treatment Form

• Client Questionnaire

• Service Contract.

Insurance

I am an out-of-network provider. I provide an invoice, which includes all of the information insurance companies require for reimbursement, at the end of each appointment. You can file these with your insurance company at any time. You, however, are responsible for paying the fee 24 hours before the session begins.
